Dosing reminders in rheumatology
- Methotrexate: reduce dose with eGFR 30–59; generally avoid below 30 (and in dialysis).
- Allopurinol: start low (≤100 mg/day; lower in stage 4–5 CKD) and titrate to the urate target — slow titration, not a fixed ceiling.
- NSAIDs: avoid in moderate–severe CKD.
- Colchicine: reduce dose in CKD; avoid with strong CYP3A4/P-gp inhibitors.
- JAK inhibitors, hydroxychloroquine and others: check the label for renal dose adjustment.
