About the BASDAI

The Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) is a short questionnaire about the past week, widely used worldwide to track ankylosing spondylitis and axial spondyloarthritis. It is scored from 0 to 10. A score of 4 or more is commonly used to indicate active disease, but your rheumatologist interprets it together with your examination, blood tests (CRP) and, often, the ASDAS score.

This is a self-help tool, not a diagnosis. It helps you keep track and talk to your doctor. If you feel very unwell, have a high fever, chest pain, sudden weakness or vision loss, seek medical care straight away.

Common questions

What is a good BASDAI score?+
Lower is better. Scores below 4 are usually taken as lower disease activity; 4 or more is commonly used to indicate active disease. The trend over time matters more than one reading.
How often should I check my BASDAI?+
Once every few weeks is enough for most people, and before each rheumatology visit. Checking daily mostly measures day-to-day variation.
Is BASDAI enough to decide on treatment?+
No. Rheumatologists combine it with your examination, CRP and often the ASDAS score. It is a useful record, not a decision on its own.
