Your diary

Trends of pain, overall well-being and joint counts. Print it before your appointment.

How to use this diary well

  • Record on a regular day each week, plus any day you have a flare.
  • Tender means it hurts when pressed or moved; swollen means it looks or feels puffy. When unsure, mark tender only.
  • The "28 joints" are the ones rheumatologists usually count in rheumatoid arthritis (shoulders, elbows, wrists, knuckles, finger joints, knees). Feet are recorded separately.
  • These are your own counts — your doctor's examination may differ. Bring the diary as a record, not as a score.

This is a self-help tool, not a diagnosis. It helps you keep track and talk to your doctor. If you feel very unwell, have a high fever, chest pain, sudden weakness or vision loss, seek medical care straight away.

Common questions

Is my diary stored online?+
No. It is saved only in this browser on this device. Use "Download a backup" if you want to keep a copy or move it to another phone.
Can my doctor calculate my DAS28 from this?+
Your counts are a useful record, but DAS28 uses the doctor's own joint examination and a blood test (ESR or CRP). Bring the diary to show the pattern between visits.
How often should I fill it in?+
Once a week on a fixed day is usually enough, plus any day you have a flare. Too-frequent entries mostly capture day-to-day ups and downs.
